Information for Professionals: Inter Agency Referral Form
To assist those working with children in making referrals, there is an inter agency referral form. This should be used either to make an initial referral about a child or young person to Social Care or to confirm a referral that you have already made by telephone in writing. This form may be posted, transmitted by fax, or sent as an email attachment and should be used in conjunction with the guidance notes.
